How it works.

Step 1
Start free. Today.

Send us your denial letter. A senior reviewer reads it within 24 hours and tells you whether it's appealable, on what grounds, and what evidence will move the needle. No credit card collected.

Step 2
We prepare and file the appeal.

Records organized, regulatory citations matched, denial criteria addressed point by point. The finished appeal is filed on your behalf — by carrier portal, fax, or certified mail. You approve before anything leaves.

Step 3
You get your money back. We get paid.

When the carrier reverses the denial and pays you, you keep 80%. We invoice 20% — capped at $5,000. Win $40,000? You keep $35,000, we take $5,000. Win $5,000? You keep $4,000, we take $1,000. If the appeal is denied at every stage, you owe nothing and we close the file.

Fair questions.

How do you decide 'recovered amount'?

The dollar amount the carrier approves or reverses as a result of the appeal — coverage, reimbursement, or reversed denial value. We calculate it from the carrier's written decision. You see the math.

What if the appeal partially wins?

Our fee applies to whatever the carrier actually pays or covers. If they approve $20,000 of a $40,000 claim, you owe 20% of $20,000.

Is the $5,000 cap really the maximum?

Yes. Even on a $200,000 recovery, our fee never exceeds $5,000. We picked the cap so a serious case doesn't punish you for winning big.

When do you bill me?

After the carrier's payment is confirmed and the funds reach you. Not before. You get an itemized invoice with the carrier's decision attached.

What if I want to walk away?

You can withdraw the case at any point before the appeal is filed and owe nothing. After it's filed, the AOB you signed remains in effect through the carrier's decision — but you still pay only on a win.

Apellica prepares and files insurance appeals. We are not a law firm and do not provide legal or medical advice. A "win" means the carrier reverses the denial in writing or pays the disputed amount as a result of an appeal we filed. Full terms on our pricing terms page.

Pricing questions

Straight answers, in plain English.

Is there really no upfront payment to start?

Yes. We do not collect a credit card at intake. Send us your denial letter and a senior reviewer reads it within 24 hours. You only pay us if we recover money for you.

How is the fee calculated?

If we win, our fee is 20% of the dollars the carrier approves or pays as a result of the appeal. The fee is capped at $5,000 per case — even on a $200,000 recovery. The carrier's written decision is the source of truth for the recovery amount.

What counts as a 'win'?

A win is when the carrier reverses the denial in writing or pays the disputed amount as a result of the appeal we filed. Partial wins count proportionally — if the carrier approves $20,000 of a $40,000 claim, our fee is 20% of $20,000.

What if the appeal is denied at every stage?

You owe nothing. We close the file, return your documents on request, and you walk away with no charge. No success-fee floor, no minimum charge, no consultation bill.

When are you actually paid?

After the carrier's payment is confirmed and the funds reach you. We send an itemized invoice with the carrier's decision attached. Payment is by ACH, check, or card — not via withholding from your recovery.

Can I cancel?

Yes. Before the appeal is filed you can withdraw at any time and owe nothing. After it's filed, the AOB you signed remains in effect through the carrier's decision, but you still only pay on a win.

Why is the cap $5,000?

We picked it so a major recovery doesn't punish you for winning big. A $50,000 recovery costs you $5,000, not $5,000+. Above $50,000 of recovery, the cap saves you money.
